A step-by-step guide on how to complete a review using Inca Evo

Instructions to complete a care plan review using Inca Evo.

Getting Started:

  • Open the patient record in your clinical software.

  • Inca Evo will automatically prompt when a care plan review is due. This prompt is based on the chronic condition, and billing history in your Practice Management Software.

  • Ensure that the patient details and information are up to date.

Starting the review:

  • Click on start to commence the review.

  • Ensure you obtain patient consent before commencing the care plan review.

Reviewing the care plan goals:

  • Review each goal from the previous care plan and update progress.

  • Use the checkbox on the left-hand side to mark completed goals.

Review the providers:

  • Keep or remove providers in the care plan review.

  • View previous number of referrals and assign new services if needed.

Finalising the review:

  • Ensure all goals and activities are properly documented.

  • Confirm any new services or referrals have been assigned.

  • Finalise the review.

Accessing a copy of the review:

  • Documentation will be available in the correspondence out folder. If documentation doesn't automatically appear, try refreshing, as seen in the below video.
